You will continue to the practice website to submit an appointment request. Appointment availability, insurance, and any referral requirements should be confirmed with the office.
New or worsening symptoms? Call 911 for new weakness, trouble speaking, seizure, severe headache, or increasing confusion or drowsiness. An appointment request is not an emergency service.
Meet Dr. Wled Wazni
Dr. Wazni is a neurologist and neurointerventionalist with fellowship training in vascular neurology, neurocritical care, and neuroendovascular surgery. His practice focuses on disorders of the blood vessels of the brain, neck, and spine.
What to bring
- Your actual CT or MRI images, as well as the radiology reports.
- Hospital discharge records and any operative or procedure notes.
- A current medication list, including blood thinners and aspirin.
- A summary of symptoms and when they began or changed.
- Insurance information and a referral if your plan requires one.
Ask the office how to transfer records securely. Do not send medical records or sensitive health details through ordinary email or a general scheduling note.

These are consultation office locations. The care team determines the appropriate setting for any procedure.
For referring clinicians
For outpatient evaluation, call 833-787-6539. Include recent imaging, symptoms, medication and anticoagulation history, previous treatment, and relevant hospital records through the practice’s approved records-transfer process.
For an unstable patient or an acute neurologic change, use emergency evaluation and the appropriate hospital referral or transfer pathway.
